HealthSpring Medicare Advantage members can get a personalized care plan after completing their 10-minute Health Risk Assessment.

What is an HRA?

Our goal at HealthSpring? To help you live a vibrant, healthy lifestyle by connecting you to the right health care resources.

The HRA is a set of 31 questions about your current health and lifestyle and helps us do just that. Filling out the HRA is easy and should take less than 10 minutes to complete.

How is the HRA used?

We use your answers to match you to the right health and wellness services for your needs. The information you provide is used to create a personalized care plan that your Care Manager shares with your health care team only, including your primary care provider.
