Patient support programs
For our members who may benefit from tailored interventions, we offer patient support programs as a supplement to your treatment plans.
The programs can provide early intervention for members’ medical, pharmaceutical and behavioral health needs. Our multidisciplinary teams support each program, which can be customized to meet members’ needs.
The programs are available at no additional cost to members. Some members may be able to self-enroll, depending on their benefit plan.

To refer a member, check eligibility or learn more, contact our care management support team.
Include in your email:
- Provider name and phone number
- Member name and health plan ID number
Use secure email when sending personal health information or diagnostic information.
We reserve the right to enroll a member in the most appropriate clinical support program.
