Review Steps for Direct Admission to Postacute Settings

HealthSpring manages prior authorization for postacute services, including skilled nursing facilities, inpatient rehabilitation and long-term acute care and home health.

For direct admissions to a skilled nursing or subacute rehabilitation facility from the community, complete the following steps before submitting a prior authorization request:

• Assess the member for medical necessity to support the need for postacute care

• Obtain a physician order and medical clearance

• Identify and confirm that there is a bed available in the accepting facility

When these requirements are met, submit a prior authorization request with required documentation. It’s quickest and preferred to submit requests through our provider portal, Availity^® Essentials.
